Example form · Quarterly service reportDekkoCORE use case
Reports to government, signed and on the record.
DekkoCORE gives health departments and the services that report to them one end-to-end encrypted place to do it: forms that come back signed, data extracts too large for email, sign-off from the chief executive, and a record of every view and download.
Before you start
This form follows one quarterly report, from a regional health service to its health department. The services, people and figures in it are fictional examples.
Part AReporting now
Less chasing, more record.
Select all that apply
Which of these are part of your reporting today?
Tick the ones you recognise. Under each is what takes its place in DekkoCORE.
Part BLodging a report
From form to row.
Follow one quarterly report from the department's template to its Responses table.
Health department
Turn the reporting template into a form.
Take the PDF you already use and turn it into a form in DekkoCORE. Drag on the fields you need, then share it with each service's reporting staff. The original PDF is never changed.
Each health service
Each service fills it in, in the browser.
Reporting staff open the form in DekkoCORE and fill it in. Each person has one response, which they can update, and no service can see another's answers.
Chief executive
Signed as it's submitted.
The signature field takes a drawn, uploaded or typed signature. On submission, DekkoCORE also signs the whole response digitally with the respondent's account key, so the department knows who sent it.
Health department
Every form comes back as a row.
Responses lists one row per submission, with who sent it, when, the signature and a column for every field. Export it as CSV for your own reporting.
Part CBoth sides
One Hub between the department and its services.

Which side of the report are you on?
What the health department does in DekkoCORE
- Build forms from the PDF templates you already use
- Give each service its own folder it can upload to
- Require a classification marking on every upload
- Follow up by encrypted mail, and take back a message sent in error
- Export responses and audit logs as CSV
Part DThe record
Every report marked, described and logged.
Files in the department's reporting Hub carry a classification marking and the same attributes, so a missing value stands out.

- activity_q1_metro.csvOFFICIAL: SensitiveQ1 2026-27Received

Part ESecurity notice
How reports are protected.
- E.1Classification markings, enforced
- Require a marking such as OFFICIAL: Sensitive on every upload and message. Microsoft Purview labels on Office and PDF files fill it in.
- E.2Every file described the same way
- Attributes such as service, reporting period and status appear as columns, and the Index flags files with a required value missing.
- E.3Hosted in Australia, IRAP assessed
- DekkoCORE is hosted in-country and IRAP assessed for Australian government PROTECTED information.
- E.4Zero-knowledge, end to end
- Reports are encrypted on the device that sends them. Form answers are decrypted in your browser, so DekkoSecure never sees them.
- E.5A record for audit
- Every view, download and signature is logged. Export the logs as CSV or stream them to your Log Analytics workspace.
- E.6Kept for as long as you set
- A data expiration policy sets a maximum file lifetime in days, so reports don't outlive your retention schedule.
Part FAttachments
Signed off, and on the log.

Part GNotes on this form
Asked by departments and services.
Note 1
Do health services need an account to fill in a form?
Yes. Form answers are signed with the respondent's account key, which is how the department knows who sent them. For a one-off file from someone without an account, a guest link can accept uploads and lasts from 3 days to 1 month.
Note 2
Can we see totals across every service?
Export the responses as CSV and total them in your own tools. There are no server-side reports or charts, because answers are encrypted in the browser and the server never sees them.
Note 3
How do services send large data extracts?
Through their folder in the department's Hub, in the browser or with the Upload Accelerator, a Windows app built for large and bulk uploads. Files land already shared with the department's reporting team.
Note 4
Which classification markings are supported?
The Australian protective marking schemes, including OFFICIAL, OFFICIAL: Sensitive and PROTECTED. A tenancy policy can require a marking on every upload and message, and Microsoft Purview sensitivity labels on Office and PDF files are read automatically.
Note 5
Can staff or the public raise a concern anonymously?
Yes, through a public submission link or QR code, with no account needed. Each submission lands in its own folder with a report of when it arrived and from what device. Because these submissions are anonymous, use forms for anything that needs a known sender.
Note 6
Where is the data stored?
DekkoCORE is hosted in-country in Australia, the United States and Canada, and is IRAP assessed for Australian government PROTECTED information.
